well VB or I think C has it's own icon creator...
if you don't have one... you can download Icon maker...(probably search in google)
And once you have the ICO file VB has the option for selecting icons

In your Form properties, find the "Icon"properties, there you will config ur Icon, must be ".ico" or ".cur" file,
then
In the Project Explorer, right click in the empty space, then click properties,
under the "Make" tab find the icon, and assign the form you assign the icon previously,
Thats it!
