have you tried to put the sd card to other phone? i think the problem is in your phone, memorycard can be put a password via phone. are you using a conveter to micro SD or not? if you are, try to unlocked it,you'll see it at the leftside of the sdcard.

This is A Weird problem but the only way you could solve this is by Reformatting the actual card.
